Winter health plan opt-out cheques are in. Visit room SC 2001 in the Student Centre weekdays between 9 AM and 4:30 PM to pick your cheque up. Bring your student card with you.
All registered full-time, post secondary students are automatically enrolled in the FSU Health and Dental Plan. This plan covers most prescription drugs and accident benefits. Some benefits of this plan include:
Full academic year coverage (September 2009 to August 31st, 2010)
Student card may be used as a pay-direct drug card (accepted at most pharmacys across Canada)
Opt-in provision for dependent coverage (within first 10 days of classes)
A new addition to the health plan is dental coverage, which includes:
100% of the cost for one cleaning, one unit of polishing and one fluoride treatment (per year)
75% of the cost for fillings
100% of the cost for one examination (including any necessary x-rays)
You can visit any dentist in Canada
Your student card is your pay direct dental card
75% coverage of extractions (including 2 wisdom teeth per year)
10% of the cost of other oral surgery (crowns, bridges, etc.)
The option to opt your family dependents in (at an additional cost)
The option to opt out and receive a refund if you have proof of similar coverage
If you pay for prescription medication while you are covered without using your student card, you can get reimbursed by filling out a form that is available in room SC2001 in the Student Centre.
