Full-time September 2010 start students only: Opt-out of health plan between Aug. 16th at 9 AM and 4 PM on September 20th

Full-time registered post-secondary students are automatically covered under the FSU Health & Dental Plan for prescription drugs, dental benefits and accident benefits. If you have similar coverage through another party you may choose to opt-out of the Health & Dental Plan. You must complete an opt-out form online at www.fsu.ca by Monday, September 20th, 2010 (by 4 PM) for the fall semester, Monday, January 17th, 2011 (by 4 PM) for the winter semester and Tuesday, May 24th, 2011 for the summer semester. (*Please note that you must opt out during your first academic semester of the school year).

Benefits of this plan include:
  • Full academic year coverage (September 2009 to August 31st, 2010)
  • Student card may be used as a pay-direct drug card (accepted at most pharmacy’s across Canada)
  • Opt-in provision for dependent coverage (within first 10 days of classes)
  • Dental coverage
Important DocumentsIf you pay for prescription medication while you are covered without using your student card, you can get reimbursed by filling out a form that is available in room SC2001 in the Student Centre.
