This application will be reviewed and if necessary taken to the Executive Council for approval. Once a decision has been reached, the club President/Vice President will be contacted.

I the undersigned, hereby acknowledge that I have read and understood the FSU Clubs Policy. By signing this waiver, I voluntarily assume all risks associated with my proposed activity or event. I understand that if my proposal violates the Clubs Policy, it will not be approved.
